Making a referral is a simple process of completing the following steps:

  1. Download and print referral pack.
  2. Complete the forms.
  3. Fax papers to the number advised in the referral pack OR post to AXA ICAS.
  4. AXA ICAS will contact you by fax, email or post with an appointment date time and location.
  5. The report will be despatched by first class post approximately 48 hours after the examination.
